Study on the current situation and latent profile analysis of nurses’ voice behavior

Objective To analyze the current situation and latent profiles of nurses’ voice behavior in nurses,and explore the influencing factors of different types. Methods Through convenience sampling,812 nurses who worked in tertiary hospitals from 5 cities in Guangdong Province during April 2022 to June 2022 were recruited. The demographic questionnaire,the Voice Behavior Scale,the Humble Leadership Scale,and the Team Psychological Safety Climate Scale were used for investigation. Latent profile analysis was used to explore the latent profiles of nurses’ voice behavior. The influencing factors of latent profiles were identified by multivariate Logistic regression. Results The total score of the voice behavior in 812 nurses is(37.04±5.88). Voice behaviors in nurses is divided into 3 latent classes,namely “conservation group”(27.83%),“balanced-risky group”(61.95%),and “the challenge group”(10.22%). The influencing factors include recruitment type,whether to assist in department management,humble leadership,team psychological safety climate(all P<0.05). Conclusion The level of nurses’ voice behavior is upper medium with heterogeneity,which can be divided into 3 latent profiles. Nurses managers should focus on nurses with conservation group and balanced-risky group and conduct early targeted prevention and care according to characteristics and influencing factors of different latent profile.

Hypertension has a high global prevalence rate,which is the main preventable risk factor for cardiovascular disease. Home blood pressure monitoring plays an important role in diagnosis,adjustment of treatment strategies and long-term follow-up of hypertension. However,there are still enormous challenges for the popularization and standardized application of home blood pressure monitoring. Therefore,this review article describes indication,standardized implementation protocol,current status and influencing factors of implementation and management forms of home blood pressure monitoring. It can provide a reference for promoting the implementation of practical guidelines for home blood pressure monitoring and optimizing the application of home blood pressure monitoring.

Analysis of influencing factors and impact path of compassion fatigue in nurses from maternity hospitals in Jiangsu

Objective To investigate the influencing factors of compassion fatigue(CF) among nurses in maternity hospitals,and then to further explore the impact path of factors. Methods 715 nurses in maternity hospitals from Jiangsu were investigated by the cross-sectional survey. A self-developed questionnaire,the Five-Factor Inventory,the Social Support Rate Scale,the Psychological Capital Questionnaire-Revision,and the Compassion Fatigue Scale were adopted to collect data. Results The number of children,personality of neuroticism and agreeableness,social support,and psychological capital are the influencing factors of CF(P<0.05),while professional titles,length of services,labor relations,personality of extraversion and openness,social support and psychological capital influence compassion satisfaction(CS)(P<0.05). In structural equation model,the indirect effects of neuroticism on CF or CS existed(ES=0.062,-0.420). Social support and psychological capital play a partially mediating role in neuroticism and CF(ES=0.017),accounting for 27.42% of the total indirect effects. They also play a completely mediating role in neuroticism and CS(ES=-0.112),accounting for 26.67%. Positive personality had a significant indirect effect on CF or CS(ES=-0.158,0.623),and social support and psychological capital serve as a completely mediating role between them (ES=-0.028,0.102),accounting for 17.72% or 16.37% of the total indirect effects. Conclusion Nursing managers should take effective measures tailored to personality traits to help nurses meet high levels of social support and psychological capital,so as to promote CS and reduce CF.

Exercise intervention in the rehabilitation of patients with lung transplantation:a scoping review

Objective To identify the specific intervention content,methods and effects of exercises,so as to provide the guidance for future research.Methods A literature search was performed in the CNKI,Wanfang,CBM,the Cochrane Library,PubMed and Embase to identify relevant studies. The included studies were summarized and analyzed.Results Totally 25 studies were included,of which 9 were randomized controlled trials and 16 were quasi-experimental studies. The results showed that exercise methods can be divided into 3 categories,namely aerobic exercise,resistance exercise and flexibility exercise. The intensity of exercise was mostly low to moderate. The frequency of exercise was mostly 3~5 times per week,lasting for 30-60 minutes per session. The results showed that exercises had positive effects on respiratory function,exercise ability,quality of life and mental health of lung transplant patients.Conclusion Exercise intervention is holistic,dynamic and comprehensive,with a positive effect on lung transplant patients. Future research should focus on exploring the best exercise intervention strategies,formulating standardized and unified evaluation standards,and formulating scientific,applicable and standardized exercise training programs for lung transplant patients based on the condition and physical function of lung transplant patients.

The status of central venous catheter closure for blood purification in ICU patients

Objective To investigate the current situation of central venous catheter closure for blood purification patients in ICUs,and to compare the differences between different hospitals. Methods In September 2020,the convenience sampling method was used to conduct an online survey of 228 ICU wards in 117 hospitals of 23 provinces. The survey included general information and the current status of the central venous catheter closure for blood purification patients in the ICUs. Results A total of 222 valid questionnaires were collected. Heparin was the most commonly used sealing liquid,followed by citric acid,and urokinase was the least. The concentration of heparin was mainly 1 250 U/ml,and the concentration of citric acid was mainly 4%. The dosage of the sealing solution was mostly 1 times the volume of the catheter;the frequency of sealing is more after each dialysis;the sealing method is mainly positive pressure combined with pulse sealing. The catheter was withdrawn before sealed;the number of withdrawals was mostly 2 times,and the withdrawal volume per time was mostly twice the catheter capacity. There are significant differences in terms of the concentration,dosage,frequency of sealing,number of withdrawals,withdrawal volume per time between tertiary hospitals and non-tertiary hospitals(all P<0.05). Conclusion The selection,concentration,dosage,frequency of the sealing of the central venous catheter for blood purification in ICUs are not the same. It is recommended to formulate the nursing standards for sealing the central venous catheter for blood purification patients in the ICU to standardize the reasonable and effective sealing of the tube by clinical nurses.

Evidence summary of non-pharmacological interventions for maternal postpartum depression

Objective To retrieve,evaluate and summarize the best evidence of non-pharmacological interventions for maternal postpartum depression and to provide references for clinical practice. Methods According to the top-down principle of the evidence-based resource 6S model,a systematic search of domestic and foreign databases and websites was conducted on the intervention of postpartum depression,including relevant clinical decision reports,best practices,guidelines,evidence summaries,systematic reviews,and expert consensuses. The search time limit was from the establishment of the databases to March 2022. A supplementary search was performed for randomized controlled trials published between September 2019 and March 2022. There were 2 researchers who evaluated the quality of the literature,extracted and summarized the evidence according to the subject. Results Totally 15 articles were involved,including 1 best practice,3 guidelines,2 expert consensuses and 9 systematic reviews. Finally,28 pieces of best evidence about iatrophysics,psychological interventions,exercise interventions,other interventions,and intervention implementation and monitoring were summarized. Conclusion Non-pharmacological interventions for maternal postpartum depression have various forms and rich contents. Appropriate intervention plans should be made according to the actual situation and the willingness of parturients,and the process of interventions should be well monitored.

Evidence summary for nonpharmacological interventions in promoting gastrointestinal function recovery after gastrointestinal malignancies surgery

Objective To comprehensively retrieve and analyze the relevant evidence of non-pharmacological interventions in promoting the recovery of gastrointestinal function after gastrointestinal malignant tumor surgery,and to summarize the best evidence. Methods According to the 6S model of evidence-based resources,computerized search in websites and databases such as UpToDate,Cochrane clinical answers,National Guideline Clearing house,the National Institute for Health and Care Excellence,New Zealand Guidelines Group,Yimaitong,Cochrane Library,PubMed,CNKI,Wanfang Data,VIP,was conducted to collect relevant clinical decisions,guidelines,expert consensuses,evidence summaries,and systematic reviews on non-pharmaceutical interventions in promoting gastrointestinal function recovery after gastrointestinal malignant tumor surgery. 3 researchers evaluated the quality of the included literature independently,and combined the professional judgment to standardize the literature information extraction. Results A total of 15 articles were included,including 4 articles on clinical decision-making,1 guideline,3 expert consensuses,and 7 systematic reviews. 12 pieces of the best evidence were formed,including topics such as multimodal intervention,preventive gastrointestinal decompression,early postoperative activities,chewing gum,drinking coffee,and acupuncture therapy. Conclusion Medical staff can follow the best evidence to formulate intervention programs to promote the recovery of gastrointestinal function for patients with gastrointestinal malignant tumors after surgery,and promote evidence-based practice.

Objective To explore the real experience of mild cognitive impairment patients with memory decline,and to provide references for clinical staff to carry out follow-up medical nursing intervention. Methods Using objective sampling method,9 patients with mild cognitive impairment in the Department of Neurology of a tertiary hospital in Beijing from May to June 2021 were selected as the research subjects for semi-structured interviews,and the data were analyzed by Colaizzi 7-step analysis method in phenomenological research. Results The real experience of memory loss in patients with mild cognitive impairment can be summarized into 3 themes:disease perception of memory loss(attitude of contempt and denial,introspection of reflection and traceability,and change of interactive relationship);emotional experience of memory decline(generation of anxiety and fear,reduction of self-efficacy,formation of self enlightening attitude,expectation of facing the future);coping strategies for memory loss (lifestyle coping,physical and mental activity coping,future planning coping). Conclusion We should strengthen the popularization of medical knowledge about memory loss in patients with mild cognitive impairment,pay attention to their emotional needs,give psychological counseling,family support and social support,and carry out personalized cognitive intervention to promote their cognitive improvement and return to family and society.

Family caregiver burden of cancer patients reduces quality of life of caregivers and quality of care provided to the patients,and it is an important factor that deteriorates the psychological well-being of cancer patients and their family caregivers. This paper reviewed the characteristics and application of domestic and foreign assessment tools for family caregiver burden of cancer patients. Besides,we analyzed the strengths and limitations of these tools,and put forward suggestions,with expectations to provide a reference for the development and application of localized assessment tools for family caregiver burden of cancer patients,and implications for future research.

Objective To systematically review and synthesize qualitative studies on the care needs of lung transplant recipients at home and to provide evidence-based guidance for the development of educational programs and the implementation of continuous care. Methods We searched databases including PubMed,Embase,Web of Science,Cochrane Library,CINAHL,ProQuest,CNKI,Wanfang Data,VIP and CBM from inception to August 2020,to collect qualitative studies exploring the care needs of lung transplant recipients at home. The Joanna Briggs Institute Critical Appraisal Tool for qualitative studies in Australia(2016 edition) was used to evaluate the quality of included studies. The results were synthesized by integrating methods. Results A total of 11 studies were included,with 60 complete research findings grouped into 11 categories by their similarities. 4 integrated findings were synthesized,namely informational needs,physiological needs,psychological and spiritual needs,and societal needs. Conclusion The care needs of lung transplant recipients at home were diverse and special. Transplantation teams should pay more attention to the care needs of this population,and provide education,monitoring,pulmonary rehabilitation,and social support for discharged recipients,in order to improve the self-management ability and their quality of life.

Construction of a nursing quality index system for cognitive training nursing clinics

Objective To construct a quality index system for cognitive training nursing clinics in line with China’s national conditions,and to provide references for related practitioners. Methods Based on the literature research,theoretical analysis,and clinical investigation,a draft on indexes was constructed. Using Delphi’s expert consultation method,16 experts were selected and 2 rounds of consultations were conducted to establish a nursing quality index system for cognitive training nursing clinics. Results The recovery and effective rates of 2 rounds of consultation documents were 100%,and the authority coefficient of the second round of consulting experts was 0.836. Expert opinion coordination coefficients of the 3-level indexes were 0.755,0.813,0.929 respectively(P<0.05). The final quality index system includes 3 first-level indicators(structure quality,process quality and outcome quality),10 second-level indicators,and 34 third-level indicators. Conclusion The quality index system of cognitive training nursing clinics constructed in this study is scientific and practical,and it can provide references for comprehensive evaluation and standardized management of cognitive training nursing clinics.

Intervention research of undergraduate intern nursing students’ professional identity

Objective To explore the influence of cognitive intervention program on undergraduate intern nursing students’ professional identity and professional self-efficacy.Methods Two comprehensive tertiary class-A hospitals which were comparable in scale,salary,hospital environment and quality of teaching staff,were selected using convenience sampling method,and were randomly assigned as the experimental group and the control group. Convenience sampling method was used to recruit 75 nursing students in the experimental group and 69 nursing students in the control group. The intervention program was based on ABC Theory and Broaden-and-Build Theory. The experimental group was given a 5-times cognitive intervention training biweekly based on the routine training. The intervention program mainly consisted of the online and offline lectures and the interactive discussion in class. The control group was given a 5-times routine professional learning during intern practice.Results A total of 138 nursing students completed the study. The overall score of nursing students’ professional identity and professional self-efficacy in the experimental group was better than those in the control group and the differences were statistically significant after the intervention(P<0.05).Conclusion The intervention program based on ABC Theory and Broaden-and-Build Theory can improve intern nursing students’ professional identity and professional self-efficacy effectively.
